Print out the patient forms you find on our site, fill them out, and bring them to your appointment along with all your current medications.

Once your doctor has assessed your cardiovascular health, he may prescribe one or a combination of the following; a special diet, an exercise regimen, a medical procedure, or additional tests. Copies of all tests and reports will be provided to your referring physician, and you can request copies for your own records, as well.
